The Committee on Vascular Lesions also attempted to correlate the appearance of lesions noted in clinical imaging studies with histological lesion types and corresponding clinical syndromes. In the histological classification, lesions are designated by Roman numerals, which indicate the usual sequence of lesions progression. The initial (type I) lesion contains enough atherogenic lipoprotein to elicit an increase in macrophages and formation of scattered macrophage foam cells. As in subsequent lesion types, the changes are more marked in locations of arteries with adaptive intimal thickening.

SummaryRecent clinical, angiographic, and pathologic evidence has prompted reexamination of the natural history of coronary atherosclerosis. Abrupt anatomic complications of atherosclerotic plaques or thrombosis on a plaque may precipitate the malignant syndromes of sudden death, acute myocardial infarction, or unstable angina before the plaque itself is large enough to limit coronary flow. This concept of a bimodal pattern of presentation has important clinical and therapeutic implications.

The work reveals critical understandings about the natural history of plaque, the correlation of vascular lesions and clinical syndromes, and the utility of new imaging methods. Diagnostic imaging atherosclerotic prevention and treatment will be shaped by these new modalities. Continuous scrutiny by the Committee on Vascular Lesions is culminating in scientific meetings every few years and is summarised in published statements. The present statement - The Vulnerable Atherosclerotic Plaque: Understanding, Identification & Modification - is the second of these documents to focus on atherosclerosis pathogenesis and pathological clinical implications.
